As it is customary with all new accounts, it is required that the attached Credit Application and the PST Exemption Certificate are completed. In August 2005 the Province of Ontario issued a new version of the "Ontario Retail Sales Tax Purchase Exemption Certificate". This certificate no longer requires a signature and if checked as "Blanket" it is valid without an expiry date until revoked by the issuer. For detailed information, you can find the complete PST Guide 204 at www.trd.fin.gov.on.ca. Submission of this completed certificate is required only once, unless company information changes. We have enclosed a copy of the certificate for your convenience; please complete and return it to our office as soon as possible.

If we do not receive a copy of the signed certificate, we must collect Provincial Sales Tax, on all purchases.
